The BCAT® Approach provides skilled nursing facilities with a variety of resources to meet the cognitive needs of a diverse patient population.  Our interprofessional, standardized, cognitive care delivery model supports assessment, intervention, and sustainable discharges from acute rehab.  The Approach also ensures the long-term care patient population have opportunities for meaningful engagement and cognitive stimulation to maintain their health and wellness.

Assessment Tools for Professionals

The BCAT® Test System is comprised of six unique assessment instruments that provide objective measures in the areas of cognition, practical judgment, and mood.  These tests can be used alone or in combination, to support comprehensive assessment, and that relevant information such as PHQ-9 / BIMS crosswalks are shared with the center's interprofessional team.

Intervention Tools for Providers

The BCAT® Approach offers a variety of evidence-based tools that can be used to improve the center’s Quality Measures,  reduce hospital readmissions, and address the behavior management needs of persons living with dementia. Our prescriptive treatment protocols right-size care for patients across the cognitive continuum and leverage a variety of evidence based interventions to support both individual or group treatment.  The utilization of the SMART™ tool (Strategic Management of Acute Rehab Transitions) can be used as a risk identification tool to ensure safe and sustainable discharges. Finally, The BCAT® Dementia Program is an evidence-based interprofessional approach to reducing dementia-related behaviors. The Program integrates three primary components: the CBS-8 for identifying and tracking dementia-related behaviors; 3D Behavior Management approach, which decodes problem behaviors and introduces specific resident-centered interventions; and MemPics® for promoting positive meaningful engagement and reducing agitation.

Brain Health Programming for Patients and Consumers
